Reference/Evaluation Forms to give to your child's teacher:

• Download the AISNE Student Evaluation Form for Grades K-1
• Download the AISNE Student Evaluation Form for grades 2-6

Please give the appropriate form to your child's teacher and have him/her return the form directly to the Atrium School via mail or fax by January 15th, 2008.

Applications and student evaluation forms are due by January 15th along with a $50 application fee.
